Is there an item on your account that you have lost?

Use this form to report your LOST item.  If there is an item on your account that you are certain has been returned, please submit a Claimed Returned Form.


  • This form will NOT delay the billing process.
  • You must fill out a separate form for each item.
  • A University Libraries staff member will contact you regarding your request.  Please check your email address before you submit.

Your library card number is at the bottom of your FSU ID and begins with 206
The item barcode is usually inside the back cover of a book and begins with 31254. You can find this in your date due email or late notice.
Please enter the date the item was lost.
